Massage Therapy LLC has Massage Client Intake Forms that need to be filled out at the time of your first visit with your consultation at no extra charge against your massage time, these will be rechecked periodically or annually. This takes approx 15 minutes, so allow enough time to fill it out, this does not count against the length of your session. The Client Intake Forms are annually reviewed to be sure they are current, & SOAP (Subjective, Objective, Assessment, Plan) documentation is noted at every visit, with or without insurance coverage, so you & your therapist/therapists can keep track how you are progressing.The attached forms listed here are for your convenience if you choose to print & bring them,  it is not necessary for them to be filled out beforehand, some will be provided. Therapists reserve the right not to perform modalities they feel are contraindications for your massage at that time, or will require a physician's referral.
